Template changelog for hosted documents

Five of the free generators can be hosted: a desk owner gets a permanent page at /p/<desk-id>/<kind> or an embed script, and the document's request section links to the desk's live form. When a template changes (a law, rule or guidance changed, or the wording improved) its version goes up here and every hosted copy re-renders on the next scheduled run, every 30 minutes, with the line "Updated <date> (template <version>): <change>".
