How do I file a copyright claim on YouTube to remove a video that uses my content?

YouTube handles copyright removals through its own webform at youtube.com/copyright_complaint_form (or YouTube Studio's Copyright page). The request must contain the DMCA § 512(c)(3) elements YouTube lists: your contact details, each infringing video's URL, a description of the work copied or a link to your original, two legal statements (a good-faith belief the use is unauthorized; under penalty of perjury, that you own or represent the copyright), and your full legal name as a signature. YouTube removes content on a valid request, gives the uploader a copyright strike, and shares certain information from your request with them; you can schedule removal to take effect in 7 days so the uploader can delete the video and avoid a strike. Three strikes in 90 days ends a channel. If the uploader counter-notifies, the video is reinstated unless you show evidence of a court action within 10 business days.
