TAKE IT DOWN Act notice and removal request form: what must they include?
The notice must tell people, in plain language, that they can request removal of intimate images or videos published without consent (including AI-generated or altered images), how to submit the request, and what happens next. Put it where intimate content could appear and on your home page.
A valid removal request identifies the content and where it is, includes a brief statement of the requester's good-faith belief that it was published without consent, gives contact information, and carries the requester's signature (typed is acceptable). You then have 48 hours to remove it and known identical copies.
Below is a ready notice you can paste, and a hosted request form that issues ticket numbers and status pages so your 48-hour clock is logged.

Notice and removal of nonconsensual intimate imagery ([Your site]) [Your site] ([your URL]) does not permit intimate images or videos of a person published without that person's consent, including images that were digitally created or altered. If you are depicted in such content on our service, or you are the parent, guardian or authorized representative of someone who is, you can ask us to remove it. You do not need an account. How to submit a request: use our request form (link this text to your Compliance Desk request page) or write to [contact email]. Please include your name and a way to reach you, the exact location(s) of the content, a brief description that lets us identify it, a statement that you believe in good faith the content was published without consent, and your signature (typed is fine). What happens next: you receive a ticket number and a status link. We remove valid reports and any known identical copies within 48 hours of a complete request, and we tell you the outcome. Reports involving minors can also be made through the National Center for Missing and Exploited Children's Take It Down service; adults can use StopNCII.org to prevent re-uploads across participating platforms. This process is provided under Section 3 of the TAKE IT DOWN Act, enforced by the Federal Trade Commission.
What the request form must collect
- Your name (or the name of the person you represent)
- Email or phone where we can reach you
- You are: the person depicted / a parent or guardian / an authorized representative
- Where the content appears: paste every URL, one per line
- Brief description that lets us identify the image or video (no need to attach it)
- Good-faith statement: “I have a good-faith belief that the identified content is an intimate visual depiction of me (or the person I represent) published without consent, and the information in this request is accurate.”
