What the notice must contain (17 U.S.C. §512(c)(3)(A))

1. Signature. A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed. Typing your full name counts.
2. The work. Identification of the copyrighted work you claim is infringed, or a representative list if one notice covers several works at the same site.
3. The infringing material and where it is. Identification of the material you want removed and information reasonably sufficient for the provider to locate it: the exact URLs.
4. Your contact details. Information reasonably sufficient to let the provider contact you: an address, a telephone number and, if available, an email address.
5. Good-faith statement. A statement that you have a good-faith belief that the use is not authorized by the copyright owner, its agent, or the law.
6. Accuracy and authority, under penalty of perjury. A statement that the information in the notice is accurate, and under penalty of perjury that you are authorized to act on behalf of the owner of the exclusive right allegedly infringed.

Steps

1. Report it on Steam Community first if a copyright form exists. Steam Community publishes its copyright process at https://steamcommunity.com/dmca/create/. A form there usually gets a ticket number faster than email; the notice text below still applies.
2. Send the notice to the designated agent. Send a notice containing all six §512(c)(3)(A) elements to Steam Community's designated agent by email dmca@valvesoftware.com. Keep a dated copy.
3. Keep the ticket and watch the clock. The statute says the provider must remove or disable access "expeditiously" once a compliant notice is received (§512(c)(1)(C)); it sets no number of hours. Keep the confirmation or ticket number and every reply.
4. Counter-notice window: 10 to 14 business days. If the uploader files a counter-notice under §512(g)(3), the provider must tell you and will restore the material between 10 and 14 business days after receiving it, unless you first notify the agent that you have filed a court action seeking to restrain the infringement (§512(g)(2)).
5. Repeat infringers. Providers keep the safe harbor only if they adopt and reasonably implement a policy of terminating repeat infringers (§512(i)(1)(A)). If the same account has infringed your work before, say so in the notice and ask Steam Community to apply its repeat-infringer policy.
