A DMCA takedown notice is a written notification under 17 U.S.C. § 512(c)(3) asking an online service provider to remove material that infringes copyright. To be effective it must include: a physical or electronic signature of the owner or authorized agent; identification of the copyrighted work; identification of the infringing material with information sufficient to locate it; the complainant's contact information; a statement of good-faith belief that the use is unauthorized; and a statement, under penalty of perjury, that the information is accurate and the sender is authorized. Providers that respond expeditiously keep their safe harbor. Section 512(f) penalizes knowing misrepresentations.
